Risk Factors and Syndromic Management of Sexually Transmitted Infections among Sex Workers in the Kadiolo Circle  [PDF]

Abstract: Sexually transmitted infections constitute a gateway for the human immunodeficiency virus by the presence of ulcerations and local inflammatory manifestations, thus increasing the risk of transmission. The aim of the study was to assess the sexual behavior and syndromic management of sexual infections among female sex workers in the Kadiolo circle. Material and method: This was a descriptive cross-sectional study carried out in the Kadiolo circle from January 1 to December 31, 2014. It focused on clandestine or posted sex workers, aged 15 and over. Results: We collected 465 sex workers aged 15 and 49, aged 26.6 on average and from Ivory Coast 52%, Nigeria 26.2% and Mali 9.7%. Educational attainment was higher with 2.4%. Posted sex workers represented 92.5% and housed in brothels with a stay of 6 months with 53.3%. They were single with 70.8% of cases and about 72.9% lived alone and without a spouse. The age at first sexual intercourse was between 15 - 19 years with 76.6% of cases. The number of first commercial sex was between 15 - 19 years (26%) and 20 - 24 years (45.8%). Usual condom use with regular sex partners was 22.1%. Condom use was almost compulsory at 89.7% with occasional clients or non-regular sexual partners. The average number of clients per day was five (5). The major reason for consultation was vaginal discharge 16.4% followed by abdominal pain. The human immunodeficiency virus test was not compulsory and only 86 workers or 18.5% accepted with a positive result of 11 people or 12.8% of cases. Conclusion: Sexually transmitted infections are common especially as they affect young people.

Abstract: Our study aims to determine diagnostic reference levels (DRL) for chest front examination in postero anterior (PA) for optimizing patient entrance surface dose (ESD) and dose-area product (DAP) of patients in west of Côte d’Ivoire. 90 patients from three hospitals undergoing conventional radiology were considered. The ESD and DAP for each patient were obtained during chest radiography (PA) examination. The measurements were performed with the device call Dose-Area Product-meter (DAP-meter) with brand Diamentor M4-KDK, type 11017. The DRL were obtained in applying the 75th percentile statistical method to the obtained ESD and DAP. The obtained DRL in ESD for chest radiography for all rooms is 0.40 mGy and in DAP is 54.85 cGy·cm2. Our DRL for ESD is higher than those obtained in Abidjan District and in other countries like UK and Cameroon. Our DRL for DAP is higher than those from Abidjan and all other countries for which a similar study was made. The comparison of these values to those from Abidjan and other countries, shows us that radiology technicians can make efforts to choose radiological parameters to reduce ESD. They must use convenable the X-rays tube to reduce DAP by reducing the patient exposure surface.
Public Effective Dose Assessment Using Gross Alpha and Beta Radioactivity Levels of Tap Drinking Water in the District of Abidjan, Cote d’Ivoire  [PDF]

Abstract: In order to assess public effective dose due to gross alpha and beta in water, 43 tap water samples were collected from different areas in the District of Abidjan. Using the low background Gas-less Automatic Alpha/Beta counting system (Canberra iMaticTM) for analysis, the gross alpha and beta concentrations found varied from 0.001 ± 0.002 to 0.063 ± 0.050 Bq/l with an average of 0.013 ± 0.012 Bq/l and from 0.067 ± 0.080 to 0.320 ± 0.120 Bq/l with an average of 0.174 ± 0.076 Bq/l, respectively in samples. The public effective dose assessment showed values of dose to ingestion of alpha and beta emitter radionuclides lower than the recommended value of dose for drinking water 0.1 mSv/y, except in 30% of the samples. These results show the need for additional studies to be conducted in order to clarify the hazardousness of these water samples. However, this study still remains important because it has provided necessary data for future tap water quality monitoring studies in the District of Abidjan.

Abstract: Drinking good quality water is essential for better health. It is therefore essential to assess the radiological quality of all water consumed in the District of Abidjan in order to prevent related hazards. Thus, the objective of this study was to assess the risk of cancer due to the ingestion of alpha and beta emitting radionuclides in the different types of water consumed in the region. A total of 63 water samples with 43 tap water samples, 5 bottled mineral water and 15 sachet water samples was collected and taken to GAEC laboratory for analysis. The low background Gas-less Automatic Alpha/Beta counting system (Canberra iMaticTM) was used to determine alpha and beta activity concentrations. Activity concentrations of both gross alpha and gross beta obtained in water sample were respectively lower than the WHO recommended limits of 0.1 Bq/l and 1 Bq/l. Also, the annual effective dose and total equivalent effective dose found in mineral bottled water samples were higher than in other types of water. The assessment of radiological lifetime risk has shown values of cancer risk due to ingestion alpha and beta emitters lower than recommended limit. These results indicate that there is no health hazard associated to consumption of water in the District of Abidjan.

Abstract: Introduction: Bladder tumours are frequent, have a poor prognosis and are the second most common genitourinary tumour in men. In Mali, recent statistical data on the prevalence of bladder tumours show a progression with 6.71% in 2006. The euro scanner is used in the diagnostic workup. The work was initiated in order to specify the role of the scanner in the diagnosis of these tumors. Patients and Methods: Prospective descriptive study was carried out in the medical imaging department of Mali Hospital from February to November 2018. Were included any patient with a bladder tumour on euro scanner. The patients had benefited from a preliminary preparation and a renal biological assessment in order to eliminate renal insufficiency. Our examinations were performed with a Siemens 16-slice scanner. These were axial slices performed without, with iodine injection from the pulmonary base to the pubic bones. Results: We had collected 17 patients out of 541 scans i.e. a frequency of 3.76%. The age range of 21 to 40 years was 58.8% with an average age of 25 years. The antecedents (ATCD) found were urinary bilharzia (47%) and smoking (17%). The clinic was dominated by haematuria. It was associated with a bladder mass (52%) and dysuria (23.52%). The CT scan showed a budding appearance, a size greater than 3 cm (82%), infiltration of peri-vesical fat (47.05%), and adenopathy (94%). Associated lesions were: utero-hydronephrosis (58.82%) and metastases (bone and liver in 11.76% of cases each). Conclusion: They are frequent and the history of urinary bilharziasis remains the main risk factor in endemic countries. CT scans are essential in the management of this disease. The diagnosis of certainty remains histology.

Abstract: Introduction: Tongue papillomas are benign epithelial tumors of the tongue lining. It is a precancerous lesion of viral origin caused by the Human papilloma virus (HPV). Objective: To describe the clinical and histological manifestations of a particular type of oral papilloma. Observation: A young woman aged 28 years resident in Siguiri (Guinea), received in consultation for a painless mass on the tongue slow evolving for 6 months duration. There was no notion of mouth bleeding. She had no particular medical and surgical history and negative for retroviral test/HIV test. Examination of the oral cavity found a fungating mass in at Right or Left posterior third of the tongue, painful on contact, sessile and pedunculated and not bleeding on contact and of soft consistency. The excision of the tongue mass was performed under general anesthesia. Biopsy was taken and sent for HPE. The consequences were mild, allowing her to be released 3 days after surgery. Histological examination showed an acanthotic stratified para-keratinized squamous epithelium containing small parts of connective tissue and koilocytes containing connective tissue nuclei reminiscent of papilloma. There was no malignancy cell. Conclusion: The papilloma of the tongue is a benign proliferative lesion characterized by slow and painless growth. I would say early diagnosis and surgical excision should be done to avoid recurrence, contact bleeding and morbidity secondary to mass effect.

Abstract: The objectives were to determine the frequency of hepatic abscess, and to describe the clinical and para-clinical aspects and the therapeutic modalities. Method and material: This is a prospective study covering 24 months (January 2016-December 2017). Results: This was a prospective study of 30 cases of liver abscess, ranging from January 2016 to December 2017, a period of 12 months. Inclusion criteria: What were included in this study were all patients hospitalized and treated for liver abscess. Criteria for non-inclusion: Abscess cases have not been treated in the service. The sex ratio was 2.3 and the average age was 35 years old. The main clinical signs were: fever (56.7%), hepatitis (73.3%) and hepatomegaly (26.7%). Hepatic collections objected to abdominal ultrasonography were localized in the right lobe in 70%. Surgical treatment was performed in 10% of cases. Medical treatment alone was performed in the majority of cases (56.7%); eclocated puncture was required in 10 cases (33.3%). The follow-up was simple in all our patients and we did not register deaths; the average duration of treatment was 5 days. Conclusion: Topical pathology in surgical practice in our country, liver abscess is common in our department; medical treatment is curative in the absence of complications.

Abstract: Objectives were to determine the frequency, describe the epidemiological and clinical aspects, therapeutic and analyze the postoperative course. Methodology: This was a retrospective study that covered 08 years (January 2009-December 2017). Inclusion criteria: all patients operated for obstruction of the small bowel by hail and/or flanging. Exclusion criteria: other types of occlusion and non-operated patients. Result: We recorded a total of 162 cases of hail obstruction by adhesions and/or flanges at 2.87%. The average age was 32.04, the sex ratio was 1.2. The average consultation time was 4 days. Abdominal pain associated with stopping of material and gas was present in all our patients. X-ray of the abdomen without preparation carried out in all the patients made it possible to objectify in 150 patients (92.6%) of the hydro-hail levels. Inoperative occlusion of hail on flange was present in 80 patients (49.4%). Occlusion of the small bowel on flange and adhesion was present in 69 patients. Adhesion obstruction of hail accounted for 6.8% (11 cases). The most commonly used surgical technique was flange resection in 91 patients (56.2%). The follow-up was simple in 151 patients (93.2%). Mortality was 1.2% of cases, i.e. 2 deaths. The average duration of hospitalization was 6 days. Conclusion: Occlusion of the small bowel by flanging and/or adherence is a surgical emergency whose prognosis depends on early management.

Abstract: Bowel occlusions by bezoars are rare pathologies. They are due to an ob-struction of the intestinal lumen by a solid mass of fruits or undigested vegetable fibers. They are seasonal (wild fruit season from March to June) in the Sahel Zone. We report a case of small bowel obstruction in a 35-year-old rural man after swallowing a significant amount of wild grapes (Lannea microcarpa). We proceeded to the resection of the occluded ileal segment and performed an ileal terminal ileal anastomosis. There weren’t any complications during the postoperative site.

Abstract:
The objective of this study was to describe the therapeutic aspects and to evaluate the surgery results of sigmoid Volvulus in the Kati (MALIA) General Surgery Department. Patients and Methods: This was a descriptive study conducted in our General Surgery Department. It took place in two phases for six years: A retrospective phase from January 1st, 2010 to December 31st, 2015 and a prospective phase from January 1st, 2016 to December 31st, 2017. This study has been approved by the Ethics Committee. It’s concerned all the patients operated in the department for colon Volvulus. Results: We collected 70 patients’ files. The Sigmoid Volvulus represented 7.8% of emergency surgery activities and 37.2% of intestinal obstruction. The male sex predominated with 98.57% against 1.43% of the female sex. The average age was 42.11 years old with limits of 18 to 70. The average deadline evolution was 2.53 days with limits of 1 to 3. The sigmoidectomy with immediate anastomosis was performed in 66% of patients, colectomy with 2-times anastomosis: according to Hartman 20% and Bouilly Volkman 11%, detorsion with colopexy 3%. The early morbidity rate was 5.71%. The average duration of hospitalization was 10.8 days with limits of 5 to 40 days. Two deaths were recorded. Conclusion: Sigmoid Volvulus is frequent in young adult sex male in Africa. We did not find any significant difference between the surgical technique and the advent of complications. The Colectomy with immediate anastomosis seems to be ideal to us that anytime the conditions are appropriate.
